By submitting this form, I am certifying that I am authorized to represent the organization disclosed below and that we agree to [1] Complete the Skill Mapping Survey with our congregation two weeks prior to the selected intensive.  [2] Allow Made to Flourish to access our congregation's vocational data to prepare an assessment report. [3] Attend in person the full time of the selected intensive. [4] Participate in follow-up video calls with our coach(es) and the practicum cohort after the intensive.
